Job Summary
As a Home Health Occupational Therapist, provide in-home occupational therapy services to help patients regain strength, mobility and independence. Administer and interpret functional tests, collaborate with the physician and multidisciplinary team to develop and revise the Plan of Care, deliver therapeutic treatment to improve strength, coordination, range of motion and functional abilities, teach compensatory strategies, and document patient progress. Participate in care coordination and discharge planning, design community reintegration and adaptive equipment solutions, train staff and family members, supervise OT Assistant and Home Health Aide performance, and ensure accurate day-of-care documentation and communication with care management. This role requires a valid driver’s license and reliable transportation.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver’s license, auto insurance and reliable transportation
