Job Summary
Occupational Therapist at CenterWell Home Health delivering in-home rehabilitation to help patients regain strength, mobility and independence. Administers and interprets diagnostic and prognostic tests of function in patients' homes, collaborates with the physician and care team to develop or revise the Plan of Care, provides therapeutic treatment to improve/restore strength, coordination and range of motion, designs retraining programs and adaptive strategies, documents patient progress, participates in discharge planning and care coordination, and may design community reintegration activities and train staff and families. Requires driving to patient locations, valid driver's license, and reliable transportation. Minimum six months of occupational therapy experience and Home Health experience are noted in the role description.
