Occupational Therapist - Rehab- Occupational Therapy

Occupational Therapist needed for CenterWell Home Health to provide in-home rehabilitation services. Responsibilities include administering and interpreting function tests in patients’ homes, collaborating with physicians and the care team to develop and revise the Plan of Care, delivering therapeutic treatment to improve strength, coordination, and range of motion, expanding retraining programs to align with behavioral goals, documenting patient progress, participating in care coordination and discharge planning, designing community reintegration and adaptive equipment strategies, training staff and family members in retraining activities, supervising Occupational Therapy Assistants and Home Health Aides, and ensuring accurate documentation and communication with supervisors and care management. Requires a valid driver’s license, auto insurance, reliable transportation, and a minimum of six months OT experience in Home Health. Salary is paid per visit/unit and reflects 40 hours per week for full-time roles within Humana's driver safety program; benefits include medical, dental, vision, 401(k), and paid time off.
