Job Summary
Provide occupational therapy services in patients\' homes to improve strength, coordination, range of motion and functional abilities; administer and interpret function assessments; develop and revise the Plan of Care with the physician and care team; deliver therapeutic treatment and instruction per physician orders; supervise and document care; coordinate with interdisciplinary team for discharge planning and community reintegration; design retraining and adaptive strategies, train staff and family members, and ensure thorough documentation of care observations and progress.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver\'s license, auto insurance and reliable transportation
