Occupational Therapist - Rehab- Occupational Therapy

Occupational Therapist in CenterWell Home Health provides in-home rehabilitation to help patients regain strength, mobility and independence. Responsibilities include administering functional assessments, collaborating with physicians to develop and revise the Plan of Care, delivering therapeutic treatments to improve strength, coordination, range of motion and daily functioning, reviewing and expanding retraining programs, documenting patient progress, participating in care coordination and discharge planning, designing community reintegration activities, training staff and family members, and supervising Occupational Therapy Assistants and Home Health Aides. Requires driving to patient homes; valid driver’s license and vehicle insurance are needed. Minimum six months OT experience and Home Health experience are required.
