Job Summary
Occupational Therapist (OT) - Home Health for Concord, Walnut Creek, Pleasant Hill, and Martinez. Provide, coordinate and oversee skilled intermittent home-based OT care; evaluate patients, develop and implement plans of care, and re-evaluate progress to maintain high-quality care. Educate patients and caregivers to promote functional status; communicate with physicians, case managers, and nursing directors; maintain accurate clinical records using company devices; travel within the service area. Requires California OT license, 1 year of home health or hospice experience, and a CA driver’s license. Required Qualifications
- Current OT License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver's License
