Deliver emotional support to patients and families while managing a flexible schedule to coordinate your own visits. This role is part of a locally owned home health, hospice, and palliative care agency with four office locations serving Santa Clara, Contra Costa, Alameda, San Joaquin, and Sacramento counties.
