Job Summary
Provide person-centered occupational therapy in SNF/long-term care settings on a PRN basis, delivering individualized treatments to maximize independence in daily activities. Conduct patient evaluations, develop and adjust treatment plans for ADLs/IADLs, assess home environments, recommend adaptive equipment, and train patients and caregivers. Maintain thorough documentation, collaborate with an interdisciplinary team, and participate in required meetings while complying with relevant policies and regulations. Strong communication and clinical decision-making skills are essential.
Required Qualifications
- Active Texas Occupational Therapy license
- In good standing with all regulatory agencies and licensing boards
- Skilled Nursing Facility (SNF) or long-term care experience preferred
- Current knowledge of occupational therapy treatment practices
- Experience with Net Health Electronic Documentation System preferred
- Ability to manage patients with varying personalities and needs
- Strong communication, collaboration, and clinical decision-making skills
