Job Summary
Conduct functional ADL retraining, adaptive strategies, transfer training, and fall prevention for a geriatric population across the SNF and Transitional Care Unit. Treat clinically complex patients, manage discharge planning and care coordination, and supervise a COTA as needed. Collaborate daily with nursing, PT, ST, and facility staff to ensure safe patient outcomes. This PRN role offers flexible hours, 2-4 hour shifts, and 2-4 days per week availability. Independence Rehab, a therapist-owned company, provides strong clinical autonomy, realistic productivity expectations, and dedicated onboarding with 1:1 training and mentorship.
Required Qualifications
- Current Utah OT license in good standing
- Degree from an accredited OT program
- Organized, self-starter who enjoys the geriatric population
- Ability to supervise a COTA
