Job Summary
In-home Occupational Therapist (PRN) providing rehabilitation and functional recovery services. Responsibilities include administering and interpreting functional tests in patients' homes, collaborating with physicians to develop/revise the Plan of Care, delivering therapeutic treatment to improve strength, coordination, range of motion and function, reviewing and expanding retraining programs, reporting patient status and progress, participating in care coordination and discharge planning, designing community reintegration activities, recommending adaptive equipment, training staff and family to implement retraining programs, supervising OT Aide and Home Health Aide performance, and thorough documentation of care observations and plans. Requires minimum six months OT experience, Home Health experience, a valid driver’s license, auto insurance, reliable transportation, and is billable per visit with pay per visit compensation. Role is part of Humana’s driver safety program and supports in-home care through CenterWell Home Health.
