Job Summary
Provide occupational therapy services to home-health patients, including administering and interpreting functional tests, collaborating with physicians and the care team to develop and revise the Plan of Care, delivering therapeutic treatment to improve strength, coordination, range of motion and function, reviewing retraining programs to align with patient goals, documenting progress and coordinating discharge planning, designing community reintegration activities, recommending adaptive equipment, training staff and families to carry out retraining programs, supervising OT staff and aides, and ensuring accurate day-of-care documentation. Requires driving to patient homes and maintaining reliable transportation. PRN/Home Health role with a focus on rehabilitation and functional improvement in patients' homes.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver's license, auto insurance and reliable transportation
