Job Summary
Occupational Therapist- PRN Home Health provides in-home rehabilitation to improve strength, mobility and independence. Responsibilities include administering and interpreting function-related assessments in patients' homes, collaborating with physicians and the care team to develop and revise the Plan of Care, delivering therapeutic treatment per physician orders to improve/restore strength, coordination and range of motion, reviewing retraining programs, reporting patient status to the physician and care team, coordinating care and discharge planning, designing community reintegration activities, recommending adaptive equipment, training staff and family members to implement retraining plans, and supervising OT staff and aides while thoroughly documenting patient care observations. Must have a valid driver's license and reliable transportation. Part-time PRN role with pay-per-visit compensation within a Humana CenterWell Home Health context.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver's license, auto insurance and reliable transportation
