Job Summary
Occupational Therapist- PRN provides in-home OT services focusing on rehabilitation and functional improvement. Responsibilities include administering and interpreting function tests in patients' homes, collaborating with physicians and care team to develop and revise the Plan of Care, delivering therapeutic treatment to improve strength, coordination, range of motion and function, reviewing retraining programs, reporting patient status to physicians and care team, participating in care coordination and discharge planning, designing community reintegration activities, recommending adaptive equipment, training staff and family members to implement retraining programs, supervising OT assistants and Home Health Aides, and thoroughly documenting observations and plans. Requires a valid driver’s license and reliable transportation. Minimum six months OT experience and Home Health experience are required. Pay per visit range is provided ($54.00 - $76.00) with additional program notes related to Humana/CenterWell benefits. CenterWell/Home Health emphasizes patient-centered rehabilitation and whole-person well-being.
