Job Summary
Home-based acute care Occupational Therapist delivering inpatient-level therapy in patients' homes within a joint Providence at Home with Compassus model. Responsibilities include comprehensive evaluations, development of individualized plans of care, discharge planning, and collaboration with families and an interdisciplinary team. Independent caseload management with home visits, coordinating with team meetings, supervising Home Health Aides and providing case management for Certified Occupational Therapy Assistants. Demonstrates adaptability to home environments, educates patients and families on safety and treatment plans, and ensures timely documentation in line with reimbursement requirements. Qualified candidates hold a state OT license, current BLS, and have 1 year of OT experience with adults in home care preferred; compensation range listed is hourly.
