Job Summary
Provide one-on-one occupational therapy to older adults in private residences, senior living communities, and via telehealth. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment focused on mobility, independence, and functional longevity. Document sessions within the FOX EMR system and collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging in place while managing your own flexible treatment schedule.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Ability to wear PPE when required
