Job Summary
Provide one-on-one occupational therapy to older adults in private residences, senior living communities, and memory care settings. Deliver evidence-based treatment plans that support mobility, functional independence, and quality of life while collaborating with interdisciplinary teams of PTs, SLPs, and fitness specialists. Perform comprehensive evaluations, develop individualized care plans, and document effectively within the FOX EMR system. Educate patients, caregivers, and staff on safety and aging in place, incorporating telehealth or virtual care when appropriate to extend access. Drive to patient locations as part of the Geriatric House Calls model, meeting physical demands including lifting and patient transfers.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
