Occupational Therapist/Physical Therapist - Physical Therapy

Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with physician plans and patient team objectives. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician, Clinical Manager, care team, and family, submitting all required evaluations, plans, progress notes, and discharge summaries. This role requires a current unrestricted Physical Therapy license and one year of experience, with compensation ranging from $59.00 to $83.00 per visit. As part of CenterWell Home Health, you will collaborate with nurses, speech-language pathologists, and home health aides to help patients recover from injury, illness, or surgery.
