Occupational Therapist/Physical Therapist - Physical Therapy

Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to guide treatment goals, then develop and implement a rehabilitation program consistent with the physician's Plan of Treatment. Provide written care instructions to PTAs and Home Health Aides, oversee their performance and documentation, and report patient progress to the care team and physician. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. Requires a current, unrestricted Physical Therapy license and one year of experience. Full-time employment at 40 hours per week with pay ranging from $59.00 to $83.00 per visit/unit. Valid state driver's license and personal vehicle liability insurance are required as this role is part of Humana's driver safety program.
