Occupational Therapist/Physical Therapist - Physical Therapy

Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals, while developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment. Provide Physical Therapist Assistants and Home Health Aides with clear written care instructions, oversee their performance and documentation, and report patient progress to the physician, clinical manager, and care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. Maintain a valid state driver's license and personal vehicle liability insurance as part of Humana's driver safety program.
