Occupational Therapist/Physical Therapist - Physical Therapy

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide clear written care instructions to PTAs and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. This role requires a current and unrestricted Physical Therapy license with one year of experience and is part of Humana's driver safety program.
