Job Summary
As a Home Health Physical Therapist, plan and administer prescribed physical therapy in patients' homes to restore function and independence. Assess and document physical strengths to guide treatment goals, develop and implement a rehabilitation program aligned with the physician's Plan of Treatment, and provide clear written care instructions and supervision to PTAs and Home Health Aides. Thoroughly document observations, interventions, and progress, and report progress to the physician and care team, including discharge summaries. Requires a valid Physical Therapy license and one year of PT experience; must have a driver's license and maintain personal vehicle liability insurance. Full-time role with compensation for base pay per visit and comprehensive benefits, located in Daytona Beach, FL.
Required Qualifications
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
