Job Summary
Perform comprehensive occupational therapy evaluations in the patient's home environment and develop individualized treatment plans based on physician orders and clinical assessment findings. Provide skilled interventions to improve functional independence, safety, and quality of life while educating patients, caregivers, and families regarding home exercise programs and adaptive equipment. Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate care, evaluate patient progress, and revise treatment goals as appropriate. Maintain complete, accurate, and timely clinical documentation in accordance with agency and regulatory requirements.
Required Qualifications
- Graduate of an accredited Occupational Therapy program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy
- Current Connecticut Occupational Therapist license
- Current BLS/CPR certification
- Valid Connecticut driver's license
- Reliable transportation and proof of automobile insurance
- Ability to travel throughout the agency service area
- Knowledge of occupational therapy principles, practices, and evidence-based interventions
- Knowledge of home health regulations, Medicare Conditions of Participation, and Joint Commission standards
- Ability to assess patient functional status and develop individualized treatment plans
- Strong clinical reasoning, problem-solving, and decision-making skills
- Ability to communicate effectively with patients, families, physicians, referral sources, and interdisciplinary team members
- Ability to work independently and manage a flexible caseload
- Ability to use electronic medical records and point-of-care technology
- Ability to drive throughout the service area in varying weather conditions
- Ability to maintain patient confidentiality and professional boundaries
Desired Qualifications
- One (1) year of occupational therapy experience
- Home health experience
- Experience with electronic medical records and point-of-care documentation
- Experience working with geriatric and medically complex patient populations
