Occupational Therapist Per Visit, Home Health Care / Nassau/Suffolk

Evaluate and assess patients using appropriate tests to establish plans of care with measurable short- and long-term goals. Implement and modify interventions based on functional status, behavioral changes, and home environment factors while collaborating with the healthcare team to optimize outcomes. Educate patients and families regarding functional status, provide home exercise programs, and instruct on safety factors. Re-assess progress toward goals and modify the plan of care accordingly. Complete accurate documentation reflecting assessments, diagnoses, and evaluations per Federal, DOH, and TJC requirements. Assist leadership with quality assessment and performance improvement initiatives.
