Job Summary
Oversee OT services by completing initial, periodic, and annual assessments of participants' functional ability and restorative potential. Coordinate with the interdisciplinary team to develop comprehensive care plans, facilitate continuity through communication with agency providers, and instruct caregivers on therapeutic activities and home modifications. Deliver individual therapy to enhance participation in activities of daily living while ensuring proper documentation and adherence to quality management programs. Assess home environments for safety adaptations and collaborate with medical equipment providers to support participant independence. Maintain active Colorado OT licensure and CPR certification while working full-time, Monday through Friday, 8-5 in Greeley, CO.
Required Qualifications
- Graduation from a college or university, with a degree in Occupational Therapy
- One year of experience as an occupational therapist
- Active Colorado OT license
- Current CPR certification
- Valid Colorado driver's license
- Proof of automobile insurance
- Ability to lift minimum of 35 lbs
Desired Qualifications
- Two years of SNF, hospital or inpatient rehab experience
- Home health experience
