Job Summary
Provide in-home occupational therapy services to improve patient independence and safety within the home environment. Conduct in-home evaluations and functional assessments; develop and implement individualized treatment plans; assist patients with ADLs (bathing, dressing, grooming, toileting); train patients on adaptive equipment and home safety modifications; complete OASIS documentation as required; collaborate with nurses, PTs, and other team members.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Active Florida OT license
- Valid driver’s license and reliable transportation
