Job Summary
Conduct in-home evaluations and functional assessments to improve patient independence and safety. Develop and implement individualized treatment plans while assisting patients with activities of daily living such as bathing, dressing, grooming, and toileting. Train patients on adaptive equipment and home safety modifications, then complete OASIS documentation as required. Collaborate with nurses, physical therapists, and other team members to coordinate care. This role requires an active Florida OT license, OASIS experience, and a Level 2 background screening in compliance with Florida AHCA regulations prior to employment. Flexible scheduling and mileage reimbursement are provided.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Active Florida OT license
- Valid driver's license and reliable transportation
- This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment
Desired Qualifications
- Home health experience preferred
- OASIS experience strongly preferred
- Strong documentation and time management skills
