Job Summary
Develop and implement occupational therapy programs for patients accessing day services by performing mobility assessments to determine physical abilities. Design individualized plans of care to maintain capability for independence, while guiding patients and caregivers in adaptive device usage and self-care activities. Monitor qualified support staff activities to ensure care plans are followed appropriately and evaluate treatment effectiveness to modify goals as needed. Conduct home, hospital, and site visits as indicated, manage required equipment and supplies, and maintain accurate treatment records and quarterly progress notes. Ensure all services adhere to regulatory standards and therapeutic settings. This role supports Neighborhood Healthcare's mission to keep seniors healthy and independent at home within a multidisciplinary team environment.
Required Qualifications
- Valid CA Occupational Therapist license in good standing
- One year experience working with the frail or elderly population
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source
- Current CPR and First Aid certification
- Ability to lift/carry 25 lbs/weight
- Ability to stand for long periods of time
Desired Qualifications
- Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with principles and methods for treatment and rehabilitation of functional impairments
- Knowledge and understanding of the Older Americans Act, Americans with Disabilities Act, and Area Agencies on Aging
- Ability to apply ingenuity and imagination in adapting activities to individual needs
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
