Job Summary
Conduct skilled nursing rehabilitation for medically complex patients, focusing on short-term rehab, discharge planning, and helping residents regain strength and independence. Collaborate with nursing, physicians, and care managers to ensure safe transitions from the Skilled Nursing Facility to home or lower levels of care. Practice at the top of your license within a well-resourced CCRC environment, utilizing evidence-based protocols and interdisciplinary collaboration to support patient recovery. Required Qualifications
- Current Florida Physical Therapy Assistant license or ability to obtain prior to start
- Interest or experience in Skilled Nursing / post-acute rehab
- Strong clinical judgment
- Teamwork mindset
- Passion for helping older adults recover and regain independence
- Must be able to practice at the top of their license
