Job Summary
PRN Occupational Therapist (OT) to provide therapy services at Woodland Manor in Elkhart, IN. Develop and implement individualized treatment plans with the rehab team, conduct evaluations with measurable short- and long-term goals, educate residents, families, and caregivers on techniques to support progress, coordinate with an interdisciplinary team, and maintain accurate documentation in accordance with regulatory standards. Requires Indiana OT license (OTR/L) and degree from an accredited OT program. Candidates should demonstrate patient-centered care, strong communication, and the ability to collaborate within a team.
Required Qualifications
- Active Occupational Therapist (OTR/L) license in Indiana
- Degree from an accredited Occupational Therapy program
- Commitment to patient-centered care and strong communication skills
