Job Summary
Occupational Therapist (OT) needed in Grand Junction, CO to evaluate residents, develop individualized treatment plans, and provide therapeutic interventions to maximize independence and quality of life in a skilled nursing/long-term care setting. Collaborates with physicians, nursing staff, and families; conducts comprehensive OT evaluations; implements individualized treatment plans; recommends adaptive equipment and environmental modifications; educates residents, families, and caregivers on techniques and safety; maintains compliant documentation; participates in interdisciplinary care plan meetings; ensures regulatory and professional standards are met.
Required Qualifications
- Current Colorado Occupational Therapist license in good standing.
- Skilled Nursing Facility, rehabilitation, or long-term care experience preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within a multidisciplinary team.
