Job Summary
Evaluate residents and develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services to residents in a skilled nursing and long-term care setting. Accurately document evaluations, treatments, progress notes, and discharge summaries. Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve the best resident outcomes. Educate residents, families, and caregivers on therapy goals and adaptive techniques. Promote resident independence, safety, and quality of life through evidence-based therapy. Follow all state and federal regulations and adhere to facility policies and procedures.
Required Qualifications
- Current Occupational Therapist (OT) license in Colorado or eligibility for Colorado licensure
- Graduate of an accredited Occupational Therapy program
- Strong communication, clinical, and documentation skills
Desired Qualifications
- Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience preferred, but new graduates are encouraged to apply
- Passion for delivering compassionate, resident-centered care
