Job Summary
Occupational Therapist (OT) to evaluate and treat patients with functional, cognitive, and ADL impairments in a Skilled Nursing Facility/Long-Term Care setting. Responsible for creating and implementing personalized care plans, collaborating with team members to ensure quality outcomes, maintaining documentation to meet regulatory requirements, and ensuring physician orders are up to date. Required Qualifications
- Licensed Occupational Therapist in the state of practice
- NBCOT certification if required by state
- Current state license as OccupationaI Therapist
- Degree in Occupational Therapy from an AOTA-accredited program (BS, MS, MA, or OTD)
