Job Summary
Occupational Therapist (OT) PRN providing patient-centered OT services in Skilled Nursing, Long-Term Care, and Home Health settings. Responsibilities include evaluating patients, developing and overseeing individualized plans of care, delivering OT treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobility, supervising OTAs/COTAs, monitoring progress, updating the interdisciplinary team, educating patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications, and maintaining accurate EMR documentation in compliance with HIPAA and regulatory requirements. Requires licensure in good standing and graduation from an accredited OT program; flexible part-time PRN schedule; opportunity to work across multiple care settings; strong clinical and communication skills; ability to work independently within a collaborative team.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in good standing (state-specific)
- Experience in skilled nursing, long-term care, or home health preferred
- Strong clinical, communication, and time-management skills
- Ability to work independently while contributing to a collaborative team environment
