Job Summary
Occupational Therapist (OT) PRN provides patient-centered OT services in Skilled Nursing, Long-Term Care, and Home Health settings. Responsible for evaluating patients, developing and overseeing individualized plans of care, and improving independence in ADLs, IADLs, cognition, strength, coordination, and functional mobility. Supervises and collaborates with OTA/COTA, monitors progress, and communicates changes to the interdisciplinary team. Educates patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications. Completes timely documentation in the EMR, ensures HIPAA/compliance, maintains a safe treatment environment, adheres to infection control, and demonstrates professionalism and respect in patient/family interactions.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in good standing (state-specific)
- Experience in skilled nursing, long-term care, or home health preferred
- Strong clinical, communication, and time-management skills
- Ability to work independently while contributing to a collaborative team environment
