Job Summary
Evaluate occupational therapy patients to determine rehabilitation needs and develop plans of care in accordance with findings. Participate in implementation and development of the Plan of Care to ensure quality, continuity, and proper discharge planning. Conduct initial therapy evaluations and re-evaluate patient needs during each visit, making follow-up assessments per OASIS and PPS guidelines. Communicate significant findings, problems, or environmental changes to supervisors, physicians, and team members. Initiate patient safety plans using patient, family, and community resources. Verify plans prior to visits and provide care according to physician orders and established standards. Instruct patients and families on activities of daily living using adaptive equipment and sensory integration skills. Assist in securing necessary equipment, including fabricating custom-made splints. Supervise and provide clinical direction to Certified Occupational Therapy Assistants, Home Health Aides, and other staff to ensure care plans are followed.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current, unrestricted OT license valid for the state of work
- Minimum one (1) year of experience in a clinical care setting
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
