Occupational Therapist (OT) - Post-Acute Rehab (SNF) - PRN

Occupational Therapist (OT) for post-acute rehab (SNF), per diem/PRN role. Responsibilities include evaluating patients for rehabilitation services within 24-48 hours of orders, developing and implementing treatment plans, supervising the OT Assistant, communicating progress with physicians and families, participating in Plan of Care and Interdisciplinary Team Meetings, maintaining documentation and billing per policy, instructing families/nursing staff, participating in discharge planning, proposing equipment to improve service delivery, and serving as a clinical supervisor for affiliated OT programs. Requires licensure in applicable states and a Master’s or doctoral level OT degree. Strong interpersonal and communication skills, adherence to professional standards, and commitment to CRI values are essential.
