Job Summary
Occupational Therapist (OT) in a post-acute rehab SNF setting. Responsibilities include evaluating patients, developing individualized OT treatment plans, delivering skilled OT services to maximize independence and function, addressing ADLs, mobility, safety, and adaptive equipment needs; collaborating with physicians, care teams, residents, and families; maintaining accurate documentation, billing, and discharge planning; and educating patients, caregivers, and staff to support positive outcomes. Required Qualifications
- Degree from an accredited Occupational Therapy program
- Active state Occupational Therapist license
- Strong communication, clinical reasoning, and patient-centered care skills
