Job Summary
Provide occupational therapy to patients unable to perform daily tasks due to illness or injury, assessing deficits, establishing goals, and documenting progress. Assist patients with exercises to improve fine motor skills and coordination, suggest adaptive equipment like grab bars, and introduce strategies to improve home safety. Educate patients and families on the plan of care, exercises, and self-care techniques. Work as part of a home health team including RNs, LPNs, CNAs, and PTs focused on the patient's plan of care and personal goals. This part-time role requires Arizona licensure, CPR certification, and a valid fingerprint clearance card, with rates ranging from $70 to $95 per visit plus mileage reimbursement.
Required Qualifications
- Current licensure or certification as an Occupational Therapist (OT) in the state of Arizona
- Graduate of an accredited Occupational Therapist program
- CPR Certification (demonstration course required)
- Valid Arizona Fingerprint Clearance Card
- Part-Time employment status
- Minimum Expectation of 5 visits or 10 hours per week
