Job Summary
Conduct occupational therapy for patients unable to perform daily tasks due to illness or injury, working within a home health team to support their plan of care and personal goals. Assess patients, observe deficits, establish therapy goals, and document progress while assisting with exercises to improve fine motor skills and coordination. Suggest adaptive equipment such as grab bars and shower chairs, evaluate fall risks, and introduce strategies to enhance home safety. Educate patients and families regarding the plan of care, exercises, goals, and self-care techniques. Provide one-on-one patient care in the comfort of the home, focusing on helping individuals regain independence and perform everyday activities.
Required Qualifications
- Current licensure or certification as an Occupational Therapist (OT) in the state of Arizona
- Graduate of an accredited Occupational Therapist program
- CPR Certification (demonstration course required)
- Valid Arizona Fingerprint Clearance Card
- Part-Time (Part time Minimum Expectation of 5 visits or 10 hours per week)
- Experience in home health preferred
