Job Summary
Perform evaluations and develop treatment plans for patients in assisted living settings, regularly re-assessing effectiveness and attending staffing meetings and family conferences. Conduct discharge evaluations to recommend home and community safety measures, providing referrals and education resources while communicating progress to the treatment team, physicians, and families. Collaborate with multidisciplinary disciplines to coordinate care and enhance patient outcomes through functional rehabilitation techniques for neuro populations. Work flexible PRN hours at approximately two days per week, practicing therapy in environments with on-site gyms, outdoor spaces, and therapeutic gardens.
Required Qualifications
- Experienced therapist interested in working with a specialized neuro population
- Provide 1:1 outpatient therapy in assisted living setting with access to on-site gym, outdoor therapeutic spaces, walking paths, gardens, and art rooms
- You value autonomy, creativity in treatment planning, and patient-centered care
- Minimum of a bachelor's degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program
- Current, unrestricted license as an Occupational Therapist by state in which practicing
- Current CPR Certification
- A minimum of one year's work experience as an Occupational Therapist
- Communicates effectively and professionally in verbal and written interaction
- Ability to lift 50 pounds
- Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
- Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
- Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
Desired Qualifications
- Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred
