Job Summary
Provide outpatient rehabilitation services in-home for the older adult population, evaluating and treating patients in private residences or in rooms within independent living facilities. Develop and implement occupational therapy programs to restore, reinforce, and enhance functional performance, adapt plans of care to individual needs, and educate patients and caregivers. Supervise therapy assistants and students as required, maintain timely documentation and billing, collaborate with the rehabilitation team, order supplies, and ensure a safe, patient-centered environment. Skills include cognitive and physical rehabilitation across mobility, balance, endurance, ADLs, and use of adaptive devices, orthotics, and prosthetics; strong case management, communication, and documentation capabilities; and ability to design and adjust treatment plans based on patient progress.
Required Qualifications
- Bachelor’s degree in occupational therapy (ACOTE-accredited)
- initial registration with the National Board for Certification in Occupational Therapy
- valid occupational therapy license in state of practice
- valid driver’s license or reliable transportation to patient residences
