Evaluate patients and develop individualized treatment plans to enhance functional independence across acute hospital, acute rehabilitation, inpatient, medical-surgical, and outpatient environments. Deliver therapeutic interventions that support daily living activities, promote safety, and maximize patient participation in meaningful routines. Collaborate closely with the interdisciplinary team, educate patients and caregivers on adaptive strategies and equipment use, and ensure timely, accurate documentation in accordance with regulatory and facility standards. Maintain a safe treatment environment while demonstrating strong communication and clinical reasoning skills to support high-quality, patient-centered care.
