Job Summary
Evaluate residents, develop and implement individualized treatment plans based on resident needs to maximize independence, function, and quality of life. Provide skilled occupational therapy services to residents recovering from illness, injury, surgery, or functional decline in a skilled nursing/long-term care setting. Assess and recommend adaptive equipment, assistive devices, and environmental modifications. Educate residents, families, and caregivers on therapeutic techniques and safety practices. Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines. Participate in interdisciplinary care plan meetings; monitor resident progress and modify treatment plans as appropriate. Ensure compliance with regulatory and professional standards while collaborating with physicians, nursing staff, and therapy professionals to achieve optimal rehabilitation outcomes.
Required Qualifications
- Current Colorado Occupational Therapist license in good standing
- Skilled Nursing Facility, rehabilitation, or long-term care experience preferred
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively within a multidisciplinary team
- Commitment to delivering exceptional resident-centered care
