Occupational Therapist (OT) | Long Term Care

Coordinate and deliver daily occupational therapy services across infant to geriatric populations, implementing individualized treatment programs and modifying intervention goals based on ongoing evaluations. Conduct direct therapy in various functional settings while gathering data from patient records, interviews, and team members to promote health and achieve treatment objectives. Prepare required documentation including evaluation results, treatment plans, and progress notes, while communicating effectively with patients, families, physicians, and interdisciplinary team members. Evaluate and monitor clinical internships with university students and participate in departmental program development and educational initiatives.
