Job Summary
Evaluates and develops occupational therapy treatment plans for patients of all ages to resume or adjust activities of daily living; collaborates with the health care team and modifies plans of care as appropriate; serves as a clinical resource and educator; conducts OT evaluations in patient homes in Urbana, IL and Vermilion County; completes comprehensive documentation including plan of care updates, goals, and discharge planning; Educates patients and families, orders supplies, and coordinates with care team; Supervises OTA and instructs health team personnel in OT phases; requires Illinois OT licensure, BLS, driver’s license, auto insurance, and is eligible for sign-on bonus and relocation if moving 50+ miles; operates within regulatory requirements and participates in case conferencing and home care protocols.
Required Qualifications
- Licensed Occupational Therapist (OT) - Illinois
- Basic Life Support (BLS) within 30 days - American Heart Association
- Driver's License - Secretary of State
- Proof of Auto Insurance - Varies
