Job Summary
Conduct home visits to adult and geriatric patients in the Denver area to perform thorough evaluations of functional status, cognitive, and motor aspects. Assess home environments for hazards, identify equipment needs, and develop individualized intervention plans to promote independent living. Continually revise care plans, educate patients and families on rehabilitative activities, and accurately document observations using electronic medical records. Requires a current Colorado Occupational Therapist license, one year of recent OT experience, and the ability to travel throughout the coverage area.
Required Qualifications
- A current Colorado Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Ability to travel throughout the office's entire coverage area
