Job Summary
Conduct home visits to adult and geriatric clients in Columbia County to perform thorough evaluations of functional status, muscle function, and cognitive aspects. Assess home environments for hazards, identify equipment needs, and develop individualized intervention plans to promote independent living. Continually revise care plans, educate patients and families on rehabilitative activities, and accurately document observations using a touch pad tablet. Requires a current Pennsylvania Occupational Therapist license, one year of recent OT experience, and ability to work independently.
Required Qualifications
- A current Pennsylvania Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Organizational skills
- Problem solving skills
- Solid computer skills
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
