Job Summary
Conduct home visits to adult and geriatric patients in Burlington, Woburn, and surrounding towns to perform thorough evaluations of functional status, muscle function, and cognitive aspects. Assess home environments for hazards, identify equipment needs, and develop individualized intervention plans to achieve favorable outcomes. Continually revise care plans, collaborate with nursing on multidisciplinary strategies, and educate patients and families on rehabilitative activities. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Earn $60-70 per point with mileage reimbursement and work as part of a multi-disciplinary team providing skilled nursing and rehabilitative care.
Required Qualifications
- A current Massachusetts Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently
- Ability to manage time effectively
