Job Summary
Conduct home health visits to adult and geriatric patients in Northumberland County, performing thorough evaluations of functional status, muscle function, and cognitive aspects to develop individualized intervention plans. Assess home environments for hazards and identify equipment needs to enhance client independence. Continually revise care plans, collaborate with nursing on multidisciplinary goals, and educate patients and families on rehabilitative care. Accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Occupational Therapist license, one year of recent OT experience, and strong interpersonal skills.
Required Qualifications
- A current Pennsylvania Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
