Job Summary
Evaluate patients' functional status, including mobility, strength, and coordination, while assessing home environments for safety risks. Develop individualized treatment plans and provide therapeutic interventions to restore function, including fabricating splints and training patients in adaptive devices. Document care accurately within the electronic medical record and communicate plans with physicians and interdisciplinary team members. Supervise Certified Occupational Therapy Assistants and oversee home health aide care when therapy is the sole skilled service. Participate in discharge planning and case conferences to support seamless transitions. Requires NBCOT registration, state licensure, and one year of clinical experience preferred.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
- Current state licensure as an Occupational Therapist
- Valid driver's license
- Reliable transportation
- Auto insurance
Desired Qualifications
- Minimum of one (1) year of clinical experience
- Home health experience
